Expelled students MUST take an initial drug screen to enter the program.
Students' Urine Drug Screen MUST be completed BEFORE enrollment.
- There is no cost to the student.
- Students must bring a picture I.D.
- Students must have this form to be tested at the clinic.

Students who are expelled due to a drug or alcohol related offense and students who test positive at an initial or exit drug screening must enroll in a drug/substance abuse program within 5 days of a positive drug test.
